Day Habilitation Assistant                $8.80/hr.

Monday – Friday, 30-35 hrs/wk. positions available at the Clifton Park, Wilton and Malta sites.  High School or GED required and experience in Human Services preferred.  General responsibilities include providing mentoring, guidance and personal direct care to participants in a classroom setting. This position is supervised by the Day Services Instructor.  Must have a valid New York driver's license and be acceptable to the ARC's insurance standards.  Other responsibilities include direct services to consumers in accordance with his/her Individual Program Plan.  He or she will provide direct assistance in all areas of individual care (including but not limited to--personal hygiene, dining and socialization skills). Additionally, the person should possess skills in training and active treatment for people in both individual and group program settings. Lastly, this person will have an understanding of behavior modification and follow behavior plans as prescribed by the interdisciplinary team.

Licensed Practical Nurse       $12.85/hr.

40 hrs/wk., Monday – Friday from 2:00 – 10:00pm in the Residential Program.  The qualifications for this position are a graduate of an accredited school of practical nursing, current New York State LPN license,  valid NYS driver's license acceptable to ARC insurance standards and at least one year experience as an LPN is preferred.  General responsibilities include that the LPN is directly responsible to the Residential Supervising Registered Nurse with respect to questions regarding Agency policies, practices or procedures.  Other responsibilities are to perform specific nursing responsibilities under the supervision of the Supervising Registered Nurse, including but not limited to: evaluating periodically the type, extent, and quality of nursing services and reports immediately to the Supervising Registered Nurse; reviewing and documenting health records and health care plans; administering medications as necessary; Informing the Supervising Registered Nurse of service recipient's health status; performing infection control monitoring of the home; assisting individuals in the carrying out of prescribed therapies or treatments as ordered by the physician and/or therapist; ordering and logging in medication from the pharmacy; maintaining formulary; checking medication cabinet weekly; reviewing monthly medexes prepared by Pharmacy one (1) week prior to new month; reviewing MAR’s daily for accuracy of documentation; ordering medical supplies; scheduling and attending medical appointments using Agency vehicle for transportation; attending in-service training; independently lifting and transferring service recipients to and from wheelchairs, bathroom facilities, therapy table, bed and performs any other transfers as required; providing hands-on direct support to the consumers residing in their assigned site; monitoring BM records and menses charts; obtaining and recording monthly weights/BP's; monitoring dietary status/Therapeutic diets; substituting at another residence during absence/illness of other nurses; and writing individual specific reports for RN’s review and signature.  Lastly the LPN must perform specific household and programmatic responsibilities under the supervision of the Supervising Registered Nurse, including but not limited to: ensuring the home is free of safety violations; implementing and documenting individual program plans as recommended by the Interdisciplinary Team; attending all staff and service recipients related meetings as requested by the Supervising Registered Nurse; and performing all other duties as requested by the Residential Supervising Registered Nurse.

Director of Residential Services

40 hrs/wk., at all of the community-based homes.  Qualifications are a Bachelors degree in human services or related field and five (5) years work experience in the administration of residential services serving persons with developmental disabilities, with two (2) years of supervisory experience; a thorough knowledge of OMRDD regulations, state and federal laws governing residential programs, and experience in program planning and development; and a valid NYS drivers license acceptable to agency insurance standards.  General duties include that the Director of Residential Services is responsible for the on-going direction, quality, and development of residential opportunities and that he/she provides direct supervision to clinical department heads and Assistant Directors of Residential Services.  Specific responsibilities are to direct Residential Services’ activities related to N.Y.S. and federal surveys, regulations, and policies; coordinates preoperational phases of program planning and development including internal and external applications, program design and implementation; communicate information regarding changes in policy, procedures, programming, staffing and regulations specific to residential services; work cooperatively with the Executive Director and Director of Finance in the annual development of the residential budget; monitor and approve all program and administrative expenditures in accordance with agency policies and procedures; ensure all expenses are consistent with the approved residential budget; recruit, supervise and evaluate share clinical staff; supervise and evaluate the job performance of Assistant Directors of Residential Services; assist Assistant Directors of Residential Services in evaluations of respective staff as needed; participate in the recruitment and selection of staff and recommends hiring of staff to the Human Resource Director; ensure that the Developmental Disabilities Profiles are completed and kept current; provide the Director of Finance with documentation necessary to submit rate appeals to OMRDD; ensure intra and inter-program collaboration in all areas of service delivery; ensure that individuals receiving residential services have an appropriate Individual Services Plan; ensure that residential premises are appropriately maintained for the health and safety of all persons receiving services; participate as a member of the policy review committee for Residential Services; participate as a member of the Special Review Committee for Residential Services; participate as a member of the Admission-Discharge Committee; act as a liaison with the county, state and federal representatives with regard to program development, program compliance, and quality assurance; prepare required and/or requested materials and report needed by the state, county and federal government; provide on-call services to the residential programs as needed; coordinate support services with Transportation, Maintenance, Business Office, Human Resources and Administration; participate in Agency in-service training and attend workshops, seminars and conferences; and perform all other duties as deemed necessary by the Executive Director.

Program Coordinator II – Day Habilitation             $14.15/hr.

Three (3) 40 hrs/wk. positions at the Wilton and Clifton Park Day Habilitation sites.  Qualifications are a            Bachelor of Science degree in Human Services, Education, Special Education, Psychology or a related field and a minimum of two (2) years work experience with individuals with developmental disabilities including one (1) year in a supervisory capacity.  General duties are to be responsible for the development, training and supervision of all programs for all assigned individuals, which includes the training, supervision and evaluation of staff in the area of implementation of programs and the ensurance of quality of programming through periodic review; coordination of all individual services directly related to their individual service plan/cfa; directly reports to the Director of the Day Services program with respect to questions regarding policies, practices and procedures; and is supervised by the Assistant Director of Day Habilitation Services or Day Habilitation Services Coordinator.  Specific responsibilities are to develop and/or review individual Service Plan (ISP) and/or Comprehensive Functional Assessment (ISP/CFA) and ensure it corresponds to the day services and residential plans, (ensures annual, physical, TB test, behavior plans, personnel protection training, self medication test, diets, medications, allergies and safeguards are included); oversee the development, training, supervision and evaluation of all classroom staff; ensure all clinical services are developed and implemented by the IDT; assist staff in the development of person centered outcomes which correspond with the individual ISP/CFA; perform individual Quality Assurance checks, ensuring active treatment and quality services; attend and participate in individual ISP/CFA meetings; assist with providing coverage for daily staffing patterns in the day services programs, routinely and or in the absence of direct care staff; assist in locating potential worksites for individuals who want employment opportunities and placement; oversee the writing of all non-clinical programs; evaluate through informal observation and formal periodic evaluation of staff's performance implementing programs, developing person-centered strategies, and developing activities which correspond with the individual’s ISP/CFA; act as a liaison with outside providers; document and ensure appropriate data collection for all program plans; ensure that monthly review of progress in all programs is completed in a timely manner; ensure that programs are revised or changed as needed; participate in in-service training on a regular basis as required; perform in-services specific to goal planning, observation, recording and data collection; review monthly calendar of activities and seek out new contact and volunteer opportunities; perform direct care duties as required or in the absence of direct care staff and perform all other duties as directed by the Director of Day Services or the Assistant Director of Day Habilitation Services.

Medical Trainer (Registered Professional Nurse)            $17.15/hr.

35 hrs/wk., requirements are that the individual must be a graduate of an accredited School of Nursing with current NYS license to practice as a Registered Professional Nurse, have one year experience in nursing, proficiency in personal computers and use of computer programs including Power Point and Word, and a valid NYS driver’s license acceptable to Agency insurance standards.  One year of nursing experience with persons with MR/DD and experience conducting staff training in the field of nursing are both preferred.  General responsibilities include that the Medical Trainer (RN) provide staff training on a variety of medically related topics and is directly responsible to the Staff Development Coordinator of SARC with regard to all Agency training needs and with regard to Agency policies and procedures.  Specific responsibilities are that the Medical Trainer (RN) conduct in-service training, including but not limited to:  Blood Borne Pathogens, Medical Topics, CPR, First Aid, Medicine Administration, and Bed Rail Safety; develop curriculums for future medical trainings as identified by needs assessment; maintain curriculums for all Agency medical in-service training; document and maintain computerized records of all staff's participation in medical training programs by entering data into ABRA Trainer software; ensure that all training materials are properly inventoried, stored, maintained and updated as needed; maintain and update schedule for all staff of all required medical in-services; ensures compliance with State, Federal, and Agency safety regulations; provide initial and ongoing training in designated curriculums for family care staff and providers; conducts pre-tests and post-tests for various training topics; conduct training at various ARC worksites at various times including evenings and weekends; communicate any changes, cancellations, or training requests to the Staff Development Coordinator; attend conferences and seminars in the area of staff training as authorized or directed; serve as a member of the Agency Safety Committee; perform the Duties of the Alpha RN as needed; administer PPD test and hepatitis vaccines as needed; and perform all other duties as requested by the Staff Development Coordinator and/or the Human Resource Director.
